How to Endorse a Support Coordinator with the NDIS
NDIS requires a participant (or their nominee/guardian) to approve and authorise a Support Coordinator before the NDIS will communicate with them or allow them to act on the participant’s behalf.
Below are the accepted methods.
OPTION 1 — Call the NDIS (Most Common & Fastest)
The participant or their nominee calls:
📞 NDIS: 1800 800 110
Provide:
- Participant’s full name
- Date of birth
- NDIS number
- Support Coordinator’s: Micheal Head
- Organisation: Albany Support Services Provider Number: 4050173214
- Phone number & email for the Support Coordinator
Say:
“I would like to endorse/nominate Albany Support Services as my Support Coordination provider so they can speak to the NDIS on my behalf.”
The NDIS will update the participant record immediately.
OPTION 2 — Complete the NDIS ‘Consent to Share Information’ Form
This form is used when the participant wants to give formal written consent for a Support Coordinator to speak with the NDIS.
Steps:
- Download the form:
- NDIS Consent to Share Information Form (from the NDIS website)
- The participant or nominee completes:
- Personal details
- Who they want to share information with (Albany Support Services)
- What information can be shared
- Sign and date the form.
- Submit it to the NDIS:
